You will develop individualized plans, monitor provider relationships, and be available for on-call support to maintain consistent care.

Qualifikationen

• Bachelor’s degree preferred.
• Minimum of two years of related work experience required.
• Previous experience working with people with intellectual disabilities is a plus.
• Valid driver’s license.
• Passion for Mosaic’s mission.
• Strong communication and relationship-building skills.
• Experience in behavior management and knowledge of regulations.
• Excellent planning and problem-solving skills.
• Computer proficiency.

Aufgaben

• Build meaningful connections with individuals, families/guardians, home providers, direct care workers, and stakeholders.
• Empower individuals to express goals, dreams, and needs with their voices heard.
• Monitor relationships to ensure positive, supportive matches.
• Develop Individual Support Plans reflecting wishes and needs within policy requirements.
• Oversee service delivery and contractual compliance; complete visits in homes or service environments.
• Provide on-call support to Mosaic at Home Providers and Direct Care Workers.
