Job summary

Mosaic is seeking a Mosaic at Home Coordinator in Colorado to build strong connections with individuals receiving support, their families, providers, and staff. You will help clients express goals and ensure voices are heard, while coordinating matches and developing personalized support plans within regulatory guidelines.

Key responsibilities include overseeing contracted services, visits in home or service environments, and providing on-call support to providers and workers.

Qualifications

• Bachelor’s degree preferred.
• Minimum of two years of related work experience.
• Experience with people with intellectual disabilities is a plus.
• Valid driver’s license.
• Strong communication and relationship‑building skills.
• Experience in behavior management and knowledge of regulations.
• Excellent planning and problem‑solving skills.
• Computer proficiency.

Responsibilities

• Build Meaningful Connections with individuals receiving support, their families/guardians, home providers, direct care workers, and other stakeholders.
• Empower individuals to express goals, dreams, and needs, ensuring their voices are heard.
• Monitor relationships between individuals and home providers or direct care workers to ensure a positive match.
• Collaborate on creating Individual Support Plans reflecting each person’s wishes and needs while meeting regulatory requirements.
• Oversee service delivery and contractual compliance; complete visits with the individual and home provider in home or service environments.
• Provide on-call support to Mosaic at Home Providers and Direct Care Workers as needed.
