Job summary

Qualifications

• Bachelor’s degree preferred; two years of related work experience required.
• Experience working with people with intellectual disabilities is a plus.
• Valid driver’s license.
• Strong communication and relationship-building skills.
• Knowledge of regulations and behavior management.
• Excellent planning and problem-solving abilities.
• Computer skills.

Responsibilities

• Build meaningful connections with individuals served, families/guardians, home providers, direct care workers, and other stakeholders.
• Help individuals express goals, dreams, and needs, ensuring their voices are heard.
• Monitor the relationships between individuals and their home providers or direct care workers.
• Develop and align Individual Support Plans reflecting each person’s wishes and needs while adhering to policy and regulatory requirements.
• Oversee service delivery and contractual compliance; complete visits with the individual and home provider.
• Provide on-call support to Mosaic at Home Providers and Direct Care Workers as needed.
