Mosaic in Denison is seeking a dedicated Mosaic at Home Coordinator/Case Manager to empower individuals with intellectual disabilities. You will build relationships with clients, families, host home providers, and direct care workers to ensure personalized, high-quality support.
This salaried role requires a Bachelor's in human services (or related field), at least two years of experience, and a valid driver's license.
Qualifications
- Bachelor's degree in human services or related field is preferred with minimum two years of related work experience.
- Valid driver's license required for home visits.
- Experience with people with intellectual disabilities is a plus.
Responsibilities
- Build epic connections with individuals, families, host home providers, and direct care workers.
- Help individuals express goals and ensure their voices are respected.
- Monitor relationships between individuals and home providers to ensure a positive match.
- Collaborate with the service planning team to create Individual Support Plans.
- Oversee service delivery and contract compliance through field visits.
- Provide on-call support to providers when needed.
