Job summary

Mosaic in Topeka, KS is seeking a Mosaic at Home Coordinator to empower individuals with disabilities by coordinating personalized supports and building strong relationships with people served, families, home providers, and staff.

You will help people express goals, monitor matches, develop Individual Support Plans, oversee contracted services, and provide on-call support, all within a compassionate, values-driven team.

Qualifications

• Bachelor’s degree preferred and at least two years of related work experience.
• Experience working with individuals with intellectual disabilities is a plus.
• Valid driver’s license is required.
• Excellent planning, organization, and problem-solving abilities.

Responsibilities

• Build meaningful connections with individuals, families, and care staff to provide exceptional service.
• Help individuals express goals and ensure their voices are respected in planning.
• Monitor relationships with home providers and direct care workers to ensure positive matches.
• Collaborate on Individual Support Plans reflecting each person’s wishes while meeting regulatory requirements.
• Oversee contracted services and complete visits in home or service environments.
• Provide on-call support to mosaic at home providers and direct care workers as needed.
