Job summary

Mosaic is seeking a Mosaic at Home Coordinator in Grand Junction, CO to empower individuals with disabilities by building strong relationships among clients, families, providers, and staff. You will help individuals express goals, monitor matches, and develop personalized support plans while ensuring regulatory compliance.

A Bachelor’s degree is preferred, with at least two years of related experience and a valid driver’s license.

Qualifications

• Bachelor's degree preferred; two years related work experience required.
• Valid Driver's license.
• Excellent communication and relationship-building skills.
• Experience with people with intellectual disabilities is a plus.
• Computer proficiency.

Responsibilities

• Build meaningful connections with individuals, families/guardians, home providers, and direct care workers.
• Help individuals express goals and needs, ensuring their voices are heard.
• Monitor matches between individuals and providers for a positive alignment.
• Collaborate to develop Individual Support Plans reflecting wishes and regulatory requirements.
• Oversee contracted services and complete visits in home/service environments.
• Provide on-call support to Mosaic at Home Providers and Direct Care Workers.
