Job summary

Benefis Health System in Montana seeks a Home and Community-Based Case Manager to support Medicaid beneficiaries in their homes and communities. You will assess psychosocial needs, coordinate services, and work with a multidisciplinary Care Management Team.

Requirements include a Bachelor's degree in social work or related field and 3 years of health care social work experience. Exemption status is non-exempt, with starting wage of $22.13 per hour.

Qualifications

• Bachelor’s degree in social work or psychology, sociology, or other field related to social work.
• Three (3) years of social work experience in a health care setting preferred.
• Knowledge of case management methods, practice, and procedure.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long-term care consumers.
• Prior knowledge of managing a budget.
• Knowledge of human behavior, disabilities, and the aging process.

Responsibilities

• Perform psychosocial assessments and coordinate psychosocial services.
• Navigate housing, financial stability, and community resources for beneficiaries.
• Coordinate with the Care Management Team to create individualized plans.
• Manage deadlines and adapt to changing situations.
• Follow licensing board requirements and Benefis Health System policies.
