Job summary

Powerback is seeking a Physical Therapist Assistant to provide outpatient rehabilitation in home settings for older adults. The role emphasizes functional, patient-centered care with supervision by a Licensed Physical Therapist and a flexible schedule across various settings.

Responsibilities include implementing treatment plans, facilitating therapeutic exercises, and collaborating with the rehab team to maximize patient independence.

Qualifications

• Must have a valid license in the state(s) of practice.
• Must have a valid driver’s license or reliable transportation to patient residences.

Responsibilities

• Attends patient care meetings and serves as a representative of physical therapy.
• Maintains adherence to treatment schedule and complete clinical and billing documentation.
• Collaborates with rehabilitation team to implement patient-specific programs.
• Assists the Physical Therapist during tests, evaluations and procedures.
• Instructs patients and caregivers in therapy techniques under supervision.
• Maintains timely documentation for all patients treated.
• Cleans and maintains equipment and reports repair needs.
• Delivers patient-centered care to support independence and functioning.
• Implements individualized treatment plans and provides treatments including therapeutic exercise, manual therapy, and functional training.
