Job summary

The Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse in Albuquerque provides direct Veteran care and care coordination within the HBPC program. You will conduct case management, assessments, triage, and Veteran education, delivering care in Veterans' homes and coordinating with the interdisciplinary team to improve outcomes.

Eligible candidates must be a graduate of a recognized nursing program and hold a current, unrestricted RN license.

Qualifications

• Graduate of a school of professional nursing approved by accrediting bodies and accredited by ACEN/CCNE; eligible for RN licensure.
• Current, full, active, and unrestricted RN license in any US state or territory.

Responsibilities

• Provide competent, evidence-based care to HBPC patients and oversee licensed staff as appropriate.
• Deliver direct Veteran care and care coordination within the HBPC program.
• Perform case management, assessment, triage, Veteran education, and care planning.
• Conduct home visits and collaborate with the interdisciplinary team to improve outcomes.
• Lead educational efforts, quality improvement, and evidence-based practice within the team.
