U.S. Department of Veterans Affairs is seeking a Home Based Primary Care (HBPC) Case Management Registered Nurse (RN).

The RN will deliver evidence-based nursing care and case management to Veterans in their homes within a 60-mile radius of the Ottumwa VA Outpatient Clinic. Responsibilities include assessment, triage, education, resource coordination, and care planning for Veterans with complex needs across primary, geriatric, preventive, and palliative care.

Qualifications

• English proficiency required for direct patient care.
• Graduation from an approved nursing program (ADN/ diploma, or BSN/ MSN depending on grade).
• Licensure as a registered nurse in a state; process may include bridge program considerations.

Responsabilités

• Provide evidence-based nursing care and case management to Veterans in their homes.
• Assess, triage, educate, and coordinate care for Veterans with complex needs.
• Plan and implement care within a 60-mile radius of the Ottumwa VA Outpatient Clinic.
