Job summary

The Home Based Primary Care Registered Nurse delivers comprehensive, patient-centered home care by coordinating education, self-management, and quality improvement. Working with an interdisciplinary team, the HBPC RN serves Veterans across the VA St.

Louis Health Care System, including Missouri and Illinois sites and rural areas. Duties include case management, assessment, triage, education for Veterans and caregivers, resource coordination, medication and treatment management, and development

Qualifications

English proficiency required for direct patient care.
Graduate of an approved nursing program with active US license.
Eligibility for state RN licensure and appropriate bridge programs if applicable.
Experience or readiness to provide home-based care and case management.

Responsibilities

Deliver and coordinate patient-centered home care with education and self-management focus.
Perform case management, assessment, triage, and care plan development.
Coordinate resources, medication and treatment management, and quality improvement.
Collaborate with interdisciplinary teams and provide education to Veterans and caregivers.
