Job summary

The New Mexico VA Health Care System seeks a Home Based Primary Care Outpatient Staff Registered Nurse for the HBPC program. This role delivers complex home-based care, coordinates with the interdisciplinary team and provides education to Veterans and caregivers.

Responsibilities include case management, triage, medication coordination and care planning to promote health in Veterans with complex needs. Schedule is Mon-Fri 7:30am–4:00pm; not virtual.

• Oversees licensed vocational nurses/nursing assistants as appropriate.
• Plans and delivers care in Veterans' homes with complex needs, focusing on health promotion and primary/palliative care.
• Leads in evidence-based-practice and collaborates with interdisciplinary teams to improve outcomes.
