The Home Based Primary Care (HBPC) Case Management Registered Nurse provides evidence-based nursing care and case management to Veterans in their homes within a 60-mile radius of the Ottumwa VA Outpatient Clinic. The nurse conducts assessment, triage, education, resource coordination, and care planning for Veterans with complex needs, including primary, geriatric, preventive, and palliative care.

Qualifikationen

• Proven nursing education and licensure.
• Experience with home health or case management is preferred.
• Ability to work with Veterans in diverse care settings.
