Veterans Health Administration, Department of Veterans Affairs, is seeking a Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse. You will provide evidence-based nursing care in the home, coordinate patient care with the HBPC team, and ensure accurate documentation of interventions and patient outcomes.

The role requires an RN license, English proficiency, and completion of an accredited nursing program. Experience in home health, VA settings, or primary care is preferred.

Formación

• English language proficiency required.
• Graduate from an accredited nursing program and licensed as an RN.
• US state RN licensure or eligibility to obtain licensure.
• Foreign-educated nurses: degree equivalency validation may be required.

Responsabilidades

• Plans and delivers technically competent care to patients in the home setting.
• Provides direct patient care and coordinates care with the HBPC team.
• Documents care in the medical record and tracks patient progress.
• Participates in interdisciplinary plan development and evaluation.
• Ensures timely and accurate documentation of nursing interventions.
