New Mexico VA Health Care System is seeking a Home Based Primary Care Outpatient Staff Registered Nurse to provide evidence-based, patient-centered care in the HBPC program. You will coordinate care for Veterans in their homes and collaborate with the interprofessional team to optimize outcomes.

You will manage caseloads, educate patients and families, and participate in quality improvement initiatives to support Veterans’ health and independence.

Formación

• Graduate of a nursing program approved by accrediting bodies (ACEN/CCNE).
• Current, full, active RN license in the United States.
• English language proficiency as required by VA.
• State nurse license or bridge program prerequisites fulfilled.

Responsabilidades

• Provide competent, evidence-based care to HBPC patients and supervise licensed staff as appropriate.
• Deliver direct Veteran care and care coordination within HBPC.
• Handle case management, assessment, triage, education, resource coordination, and care planning.
• Plan and deliver home-based care for Veterans with complex needs including health promotion and palliative care.
• Coordinate medications and treatments and contribute to quality improvement.
• Play a role in educating staff, ensuring safety, and fostering evidence-based practice.
