Job summary

The U.S. Department of Veterans Affairs seeks a Registered Nurse to provide direct patient care in Veterans' homes through the Home Based Primary Care (HBPC) program.

The role emphasizes patient education, self-management support, and care coordination in collaboration with an interdisciplinary team. Responsibilities include administering medications, following established policies, and ensuring quality outcomes for Veterans across the continuum of care.

Qualifications

• English language proficiency required for direct patient care.
• Graduate of approved nursing program (ACEN/CCNE accredited).
• State licensure as registered professional nurse upon appointment; GNT exception may apply.
• Foreign nursing graduates require equivalency validation and active RN registration.
• Pre-placement physical and immunization requirements.
• Family/patient education and care coordination are core expectations.

Responsibilities

• Provide direct patient care in home-based primary care (HBPC).
• Administer medications and procedures per policies.
• Coordinate care with interdisciplinary team to improve outcomes.
• Lead patient education, self-management, and satisfaction across care continuum.
• Mentor staff and oversee nursing assistants as appropriate.
• Perform triage, assessment, and care planning for Veterans.
