Job summary

HarmonyCares is hiring Nurse Practitioners to deliver in-home primary care for high-acuity patients across selected markets. This role emphasizes patient-centered care delivered in the patient’s home, utilizing an integrated care model funded by HarmonyCares Medical Group.

Clinicians will work to address complex medical needs with time and attention to each patient’s circumstances, supported by a team-based approach and company resources.

Qualifications

• Licensed NP in the state where HarmonyCares Medical Group office located.
• Federal DEA license/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide home-based primary care with a focus on patient-centered care.
• Deliver comprehensive assessments and management for high-acuity patients at home.
• Balance patient time with thorough needs assessment and care planning.

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
