Job summary

HarmonyCares is seeking a Nurse Practitioner to provide in-home primary care to high-acuity patients in select markets. The role emphasizes patient-centered care, thoughtful assessment, and a collaborative approach with the care team.

Applicants must be a licensed NP with DEA eligibility, geriatric experience, CPR/BLS certification, and a valid driver’s license. This full‑time position offers competitive base pay and comprehensive benefits, with some markets providing a company car and support

• Geriatric training/experience preferred.
• Active CPR/BLS certification.
• Board certification as NP.

Responsibilities

• Provide primary care to high acuity patients in the home setting, Monday to Friday.
• Deliver patient‑centered care with time to address each patient’s needs.
• Collaborate with interdisciplinary team to coordinate home health services.

The Nurse Practitioners we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
