Job summary

Bloom Healthcare is a physician-led, employee-owned practice delivering high-touch primary care to homebound seniors in the Dallas/Fort Worth area. As an NP or PA you’ll provide relationship-based care in patients' homes and communities, supported by an interdisciplinary team and advanced data systems.

Qualifications

• NP or PA with a master's degree and active Missouri licensure.
• Passionate about serving seniors and medically complex populations.
• Skilled at building trust, communicating clearly, and adapting to home-based environments.
• Motivated by mission, relationship-driven care, and meaningful outcomes.

Responsibilities

• Provide longitudinal primary care in patients' homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with support from the care team.
• Address social determinants of health and access barriers.
• Utilize Bloom’s data ecosystem to track health metrics and coordinate timely follow-ups.
