Job summary

Bloom Healthcare in Aurora, CO is hiring a Primary Care NP/PA to deliver in-home and community-based primary care. You’ll work autonomously with a strong inter-disciplinary team in a patient-centered model designed for medically complex seniors.

You’ll see a defined panel, manage chronic and acute conditions, and use Bloom’s data ecosystem to improve outcomes while collaborating with MDs, nurses, schedulers, and care coordinators.

Qualifications

• Master's degree and active Colorado licensure as an NP or PA.
• Experience delivering primary care in home-based settings is preferred.
• Ability to work independently with strong communication skills.

Responsibilities

• Provide longitudinal, holistic medical care in patient homes and communities.
• Diagnose and manage chronic and acute conditions with care team support.
• Address social determinants of health and access barriers.
• Conduct wound care, transitional care, and condition-specific assessments.
• Perform comprehensive evaluations, medication reconciliation, and education.
• Coordinate post-discharge visits and track outcomes with analytics.
