Job summary

Ennoble Care is seeking an experienced Advanced Provider (NP or PA) to deliver primary and palliative care to homebound patients in the Jackson, Mississippi region on a full-time basis. You will work under the supervision of a physician and care for patients with chronic diseases in a home setting.

You'll conduct physical exams, update medical histories, develop care plans, and coordinate with a multidisciplinary team to ensure high-quality, patient-centered care.

Qualifications

• Board Certified, with a valid NP or PA certification in Mississippi.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.
• Strong clinical skills to assess, diagnose and treat patients.

Responsibilities

• Provide high quality care to primary and palliative patients in and around Jackson, Mississippi.
• Assess patient's health by performing physical exams, updating medical histories, and creating care plans.
• Communicate with office staff and clinical operations across internal platforms.
• Provide patient-centered care to a panel of patients.
• Develop impressions, diagnoses, and treatment plans.
• Foster trust and consult with patients, families, and caregivers during visits.
• Order diagnostic testing as medically necessary.
• Prescribe medications and medically necessary interventions.
