Job summary

Bloom Healthcare seeks a Nurse Practitioner or Physician Assistant to provide primary care in home and community settings in Centennial, CO. You will collaborate with an interdisciplinary team to deliver relationship-based, high-quality care for medically complex patients, with autonomy and strong support.

The role emphasizes independence within a team, with a focus on aging in place, social determinants of health, and measurable health outcomes.

Qualifications

• Master’s degree and active Colorado licensure.
• Experience serving seniors or medically complex populations a plus.
• Ability to work independently with team support in home-based settings.
• Comfort practicing medicine in home-based environments.

Responsibilities

• Serve as primary care provider for a defined patient panel in homes and communities.
• Diagnose, manage, and treat chronic and acute conditions with support from MD leadership and operations.
• Address social determinants of health, including housing, nutrition, caregiver support, and access barriers.
• Conduct specialty visits such as wound care and post-hospital transitional care.
• Complete comprehensive evaluations including medication reconciliation, fall risk assessments, and advance care planning.
• Utilize Bloom’s data ecosystem to track hospitalizations and coordinate timely post-discharge visits.
• Collaborate with internal and external care teams to ensure continuity and reduce readmissions.
