Abode Care Partners is seeking a compassionate Nurse Practitioner to provide primary care in seniors’ homes and other living communities. You will develop personalized care plans, manage chronic conditions, and collaborate with interdisciplinary teams to ensure seamless transitions and better outcomes.

Qualifications

• Active state NP license.
• Current NP National Board Certification.
• Active state DEA licensure.
• MSN degree.
• 5+ years nursing experience; at least 2 years NP experience preferred.
• Strong interpersonal communication skills.
• Excellent time management skills.
• Proficiency in medical instruments and equipment; EMR systems.

Responsibilities

• Plans, directs, and evaluates patient medical needs.
• Examines and treats chronic and acute illnesses, minor injuries.
• Orders and interprets laboratory and diagnostic testing.
• Collaborates with caregivers and other healthcare professionals.
• Provides referrals when necessary.
• Keeps patient medical records up to date.
