Job summary
Abode Care Partners is seeking a compassionate Nurse Practitioner to deliver primary care to seniors and complex patients in their homes across Louisville. You will perform clinical assessments, develop personalized care plans, and collaborate with interdisciplinary teams to improve outcomes.
The role offers a revamped compensation plan with base salary plus quarterly performance incentives, a comprehensive benefits package, professional growth, and flexible scheduling with weekday daytime
Qualifications
- Active state NP license required.
- Current NP National Board Certification.
- MSN or higher in Nursing.
- DEA licensure required.
- Five+ years nursing experience; NP experience preferred.
- Strong communication and time management.
- Experience with EMR and HIPAA/OSHA regulations.
Responsibilities
- Perform clinical assessments and develop care plans for seniors in home settings.
- Treat chronic and acute conditions with evidence-based care.
- Order and interpret labs and tests as needed.
- Collaborate with care teams to coordinate transitions.
- Maintain up-to-date medical records.
- Provide referrals when necessary.
