You will conduct assessments, develop care plans, and collaborate with a multi-disciplinary team to improve patient outcomes while offering incentives, growth opportunities, and comprehensive benefits.

Qualifications

• Active NP license in state
• Board certification for NP
• DEA licensure
• MSN degree and NP training
• 5+ years nursing experience; 2+ years NP preferred
• Strong communication and time management skills
• Proficiency with medical instruments and data systems
• HIPAA/OSHA knowledge

Responsibilities

• Provide primary care in skilled nursing, assisted living, and home settings
• Conduct clinical assessments and create personalized care plans
• Manage chronic conditions and acute visits focusing on prevention
• Coordinate with interdisciplinary teams for seamless care transitions
• Document patient medical records accurately
• Refer to other providers when needed
