Job summary

Abode Care Partners is seeking a compassionate Nurse Practitioner to deliver primary care to seniors and patients with complex needs in homes, skilled facilities, and communities in Ohio. The role emphasizes autonomy, comprehensive care planning, and collaboration with interdisciplinary teams.

Qualifications

• Active state NP license
• Current NP National Board Certification
• Active state DEA licensure
• Master’s degree in Nursing (MSN)
• Five+ years nursing experience; at least two years NP experience preferred
• Strong interpersonal communication skills (verbal & written)
• Excellent time management and daily workflow accountability
• Proficiency with medical instruments and equipment
• Knowledge of HIPAA/OSHA regulations
• Experience with computer-based data management, EMR, and MS Office
• Licensed driver with valid auto insurance
• Ability to engage in moderate physical activity (lifting up to ~25 lbs, standing/walking longer shifts)

Responsibilities

• Performs evidence-based medical care by planning, directing, and evaluating patient medical needs
• Examines and treats chronic and urgent illnesses, minor injuries, and other conditions
• Orders and interprets laboratory and diagnostic tests as needed
• Collaborates with caregivers and other healthcare professionals to provide comprehensive care
• Provides referrals to other healthcare practitioners when necessary
• Maintains up-to-date patient medical records
