Qualifications

• Master's degree and NY-approved NP program.
• New York NP license/NY State certification.
• NPI and DEA registration to prescribe controlled substances.
• Experience in nursing or related medical field.
• Excellent communication and leadership abilities.

Responsibilities

• Provide in-home palliative care assessments and management.
• Perform home visits to assess status and manage symptoms.
• Collaborate with physicians, social workers, and staff to develop care plans.
• Coordinate transitions with home health, hospice, and hospital teams.
• Document clinical notes in accordance with policies.
• Engage in weekend/on-call coverage as needed.
