The role centers on delivering high-quality, patient-centered care in patients' homes, with collaboration from physicians, nurses, social workers, and hospice partners to develop individualized care plans.

Ideal candidates have experience in palliative and hospice care, home-based primary care or house calls, and managing advanced chronic illness.

Qualifications

• Experience in palliative and hospice care, and home-based primary care.
• Ability to develop individualized care plans in collaboration with the care team.
• Comfortable working independently in patients' homes.
• Experience with advanced chronic illnesses and complex symptom management.

Responsibilities

• Provide home-based palliative care visits across the community.
• Collaborate with physicians, nurses, social workers, hospice partners, and other team members.
• Develop individualized care plans aligned with patient goals and values.
• Support goals-of-care and advance care planning conversations.
