Qualifications

• Current, unrestricted Registered Nurse license in New York State.
• BSN preferred and accredited nursing program graduate.
• Minimum of 2 years of professional pediatric nursing experience.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication, organization, and care-coordination abilities.
• Valid driver’s license and ability to travel throughout the community.
• Ability to work independently within an interdisciplinary team.
• Commitment to family-centered, compassionate, culturally responsive care.

Responsibilities

• Provide skilled nursing care to pediatric hospice patients at home and in community settings.
• Perform initial and ongoing nursing assessments of physical, developmental, emotional, and psychosocial needs.
• Develop and update individualized plans of care with the multidisciplinary team.
• Manage pain and complex symptoms and educate caregivers on treatments.
• Coordinate admissions, discharges, transitions of care, and changes in level of care.
• Collaborate with physicians, social workers, and other team members to ensure continuity of care.
• Provide end-of-life care and family support.
