Job summary

System Support seeks an Occupational Therapist Assistant to deliver in-home occupational therapy services under physician orders and supervision. You will help implement the plan of care, document care provided, and communicate with the health care team for coordinated patient progress.

Position requires Ohio OTA license and CPR certification, with preferred experience in acute care within the last three years. Strong caregiver communication and documentation skills are essential.

Qualifications

• Associate degree required.
• Ohio OTA license and CPR certification.
• One year acute care experience preferred but not required.

Responsibilities

• Provide occupational therapy services in patients' homes or residences as ordered by physician and supervised by Occupational Therapist.
• Document all care and communicate pertinent information to the health care team.
• Implement and report on the plan of care, under supervision and per facility policies.
