Job summary

PowerBack Rehabilitation in Concord, NH is seeking a licensed Occupational Therapist to provide in-home outpatient therapy for the aging population, serving patients in private homes and assisted living settings. This salaried role offers flexible schedules and diverse clinical settings across the region.

You will lead patient-centered treatment plans, supervise support staff, coordinate with the rehab team, and ensure high-quality care with documentation and safety at the forefront.

Qualifications

• Bachelor’s degree in ACOTE-accredited OT program.
• Initial registration with NBCOT.
• Valid, unencumbered OT license in NH.
• Valid driver’s license or reliable transportation.

Responsibilities

• Provide in-home outpatient OT services for older adults.
• Supervise OTAs, temporary licensees, students and support staff as allowed.
• Adhere to treatment schedule and complete clinical/billing documentation.
• Attend patient care conferences and related meetings.
• Refer to other disciplines to address comprehensive patient needs.
• Collaborate with rehab team to select appropriate regimen.
• Adapt plan of care to individual patient needs.
• Maintain safe home and clinical environments; address hazards.
• Order supplies and maintain equipment; report repairs.
• Deliver patient-first care with compassionate service.
• Assess patients, establish goals, implement therapy, document progress.
• Design/fit adaptive devices and train patients.
• Instruct patients in functional activities and monitor progress.
• Perform re-examinations and adjust plans as needed.
• Perform other related duties as required.
