Job summary

Ennoble Care in Levittown, PA is seeking an experienced Nurse Practitioner to provide primary and palliative care to homebound patients in Bucks County. You will apply medical principles under physician supervision, caring for patients with chronic diseases and coordinating care with a diverse team.

Requirements include Board Certified NP with PA license, 1–2 years in primary care with geriatric patients; hospice experience is a plus.

Qualifications

Board Certified NP with PA license.
1–2 years of primary care experience with geriatric patients.
Hospice experience is a plus but not required.

Responsibilities

Provide high-quality primary and palliative care to homebound patients in Bucks County, PA.
Assess health, perform examinations, update medical histories, and plan care.
Communicate with office staff and clinical operations across platforms.
Care for a patient panel with high-quality, patient-centered care.
Attend quarterly medical staff meetings.
Develop impressions, diagnoses, and treatment plans.
Consult with patients, families, and caregivers during visits.
Order diagnostic testing as medically necessary.
Prescribe medications, supplies, and appropriate interventions.
