Your Health is seeking a Nurse Practitioner to provide patient-centered care in patients' homes within the Blythewood area. You will collaborate with an interdisciplinary team to evaluate high-risk patients, coordinate care plans, and manage care across homes, clinics, and telehealth.

This full-time role offers a salary-based base with a monthly bonus potential and opportunities to contribute to quality outcomes and efficient care delivery. Travel between offices and care settings is required.

Qualifications

• NP/PA/Physician license in good standing in the state of employment.
• Experience in home health, clinics, facilities, telehealth, or specialty care.
• Strong communication and teamwork for interdisciplinary care.
• Ability to evaluate high-risk patients and create care plans.
• Commitment to compassionate, patient-centered care and excellence.
• Strong organization and multitasking abilities.
• Must travel between offices and locations as needed.

Responsabilités

• Evaluate high-risk patients with complex needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team for continuity of care.
• Communicate with case managers, coordinators, social workers, and physicians to coordinate care.
• Participate in team meetings to discuss progress and plans.
• Foster communication to enable seamless patient management.
• Meet efficiency expectations while delivering high-quality home/clinic care.
• Document care activities accurately to support plans and compliance.
• Educate patients with resources for conditions and self-management.
• Guide patients and families to understand options and participation.
• Adhere to HIPAA, Medicare, and related regulations.
• Monitor quality measures and pursue improvement opportunities.
