Qualifications

• Active license in the state of employment (NP/PA/MD).
• Experience in home health, clinic, facility, telehealth, or specialty care settings preferred.
• Strong communication and teamwork skills.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with interdisciplinary team members.
• Communicate with case managers, social workers, pharmacists, and others to coordinate care.
