Job summary

Paycom is seeking a Hospice RN Case Manager to administer nursing care for patients in their homes and coordinate with the Interdisciplinary Group/Team. The role includes travel within the North Dallas area and on-call responsibilities to ensure quality patient care.

Requirements include an active RN license, CPR, and a minimum of two years of clinical nursing experience, with hospice experience preferred. Day shift, full-time position in Plano, TX.

Qualifications

• RN licensure in good standing with state board.
• CPR certification and ability to read/write English.

Responsibilities

• Initial and ongoing comprehensive patient assessments.
• Develop, implement, and update the plan of care.
• Educate patients/families on disease process, care strategies, and end-of-life options.
• Coordinate care with the Interdisciplinary Group/Team (IDG).
• Provide on-call coverage and responsive communication to the team.
