Job summary

Hospice Care Partners in Irving, Texas is seeking a Hospice RN Case Manager to lead the plan of care for a caseload of patients in the North Dallas / Garland area, across homes, assisted living communities, and nursing facilities.

You will report to the Director of Nursing and collaborate with the Medical Director, social workers, chaplain, and hospice aides to coordinate compassionate, patient‑centered care.

Qualifications

• Active Texas or compact multistate RN license in good standing.
• Hospice or home health experience preferred; strong med-surg or acute care experience considered.
• Sound clinical judgment and comfort working independently in the field.
• Strong documentation habits and follow-through.
• Dependable, with reliable transportation, a valid driver’s license, and current auto insurance.
• Consolo EMR experience is a plus.

Responsibilities

• Complete admission and comprehensive assessments, and update the plan of care through recertification.
• Case manage assigned patients: symptom management, ongoing visits, and timely care plan updates.
• Support and educate patients and families, including guidance through the end-of-life process.
• Participate in weekly IDT meetings and coordinate care across the team.
• Communicate with facility staff and referral partners in your territory.
• Document accurately and on time in our EMR (Consolo).
• Participate in on-call coverage [rotating weekends].

About Hospice Care Partners

Hospice Care Partners (HCP) is a Medicare-certified hospice agency based in Irving, Texas. We are a hands-on, growing team focused on comfort, dignity, and responsive support for patients and families. We are expanding into North Dallas and building a strong clinical team under our new Director of Nursing.

The Role

As a Hospice RN Case Manager, you will lead the plan of care for a caseload of hospice patients in the North Dallas / Garland area, in homes, assisted living communities, and nursing facilities.
