Job summary

Hospice Care Partners in Irving, TX seeks an experienced Hospice RN Case Manager to lead the plan of care for a caseload in the North Dallas / Garland area. You will work in patients’ homes, assisted living communities, and nursing facilities under the direction of the Director of Nursing.

You will conduct admissions, manage symptoms, educate patients and families, and collaborate with the care team. A Texas RN license and reliable transportation are required.

Qualifications

• Active Texas or compact multistate RN license in good standing.
• Hospice or home health experience preferred; strong med-surg or acute care experience considered.
• Sound clinical judgment and independence in the field.
• Strong documentation habits and follow-through.
• Dependable with valid driver’s license and auto insurance.

Responsibilities

• Complete admission and comprehensive assessments; update plan of care through recertification.
• Case manage caseload: symptom management, ongoing visits, care plan updates.
• Support and educate patients and families through end-of-life process.
• Participate in weekly IDT meetings and coordinate care across the team.
• Communicate with facility staff and referral partners in the territory.
• Document accurately and on time in EMR (Consolo).
• Participate in on-call coverage on rotating weekends.
