Job summary

Signature Albany Hospice is seeking a skilled Registered Nurse (RN) Case Manager to deliver high-quality hospice care to patients in their homes in Albany, OR. This full-time role requires experience in hospice, home health, and care coordination, working independently in the field while collaborating with an interdisciplinary team.

Responsibilities include comprehensive nursing assessments, care planning, supervision of HHAs/CNAs/LPNs, clinical documentation, and on-call participation.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license - Oregon.
• Minimum 2 years nursing experience (hospice, home health or public health preferred).
• Current CPR certification.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Perform comprehensive nursing assessments and develop individualized plans of care.
• Coordinate and manage hospice services in collaboration with physicians and care teams.
• Provide skilled nursing care, treatments, and patient education.
• Monitor patient progress and communicate changes in condition to physicians.
• Supervise and evaluate Home Health Aides (HHAs), CNAs, and LPNs/LVNs.
• Participate in IDG, quality assurance and performance improvement programs.
