Job summary

Signature Eugene Home Health is seeking a full-time RN Case Manager in Eugene, OR. You will perform comprehensive nursing assessments and develop individualized care plans, coordinating home health services with physicians and care teams.

You will supervise HHAs/CNAs/LPNs, ensure accurate documentation, and participate in on-call rotations. A valid Oregon RN license and CPR are required, with at least 2 years in home health or public health preferred.

Qualifications

Graduate of an accredited nursing program.
Active RN license - Oregon.
Current CPR certification.
Minimum 2 years nursing experience (home health or public health preferred).

Responsibilities

Perform comprehensive nursing assessments and develop individualized plans of care.
Coordinate and manage home health services in collaboration with physicians and care teams.
Provide skilled nursing care, treatments, and patient education.
Monitor patient progress and communicate changes in condition to physicians.
Identify and coordinate discharge planning needs.
Supervise HHAs, CNAs, and LPNs/LVNs and ensure quality care delivery.
