Job summary

Community Hospice in Albany, NY is seeking a full-time RN to join the Nursing Home Admissions Team across nine counties. The role involves admitting patients to hospice services, coordinating with physicians and hospice directors, and ensuring smooth handoffs to the Case Manager.

Travel to partner nursing homes is required. The ideal candidate holds NY RN licensure, an Associate Degree in Nursing (BSN preferred), at least 1 year of RN experience (long-term care or homecare preferred), hospice

Qualifications

• Current license to practice as a Registered Nurse in the State of New York.
• Associate's Degree in Nursing; BSN preferred.
• 1 year experience as an RN required; long-term care or homecare preferred.
• Hospice experience preferred.
• Valid Driver's License and reliable vehicle.
• Excellent communication skills; ability to communicate with patients, physicians, and peers.
• Strong computer skills.
• Strong organizational and time management skills.

Responsibilities

• The RN coordinates with the patient's community/nursing home physician and hospice medical directors to confirm eligibility for hospice services.
• The workday includes two admissions to Hospice.
• Go to nursing homes to complete the initial assessment of patient/family needs and begin implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team.
• Ensure a smooth hand-off to the Hospice Case Manager.
• Admissions are across nine counties; region determines patient assignment.
