Job summary

Department of Veterans Affairs (HBPC) seeks a Staff Registered Nurse to deliver foundational, age-appropriate care in home settings. You will assess, plan, implement, and evaluate care while coordinating education, self-management, and patient satisfaction across the continuum of care.

Responsibilities include medication administration, care planning, and collaboration with an interdisciplinary team to optimize outcomes for Veterans with complex needs.

Qualifications

• Must possess current, unrestricted RN license in the United States.
• Excellent written and spoken English proficiency.
• Experience in home-based primary care or VA settings preferred.

• Assess, triage, educate, and coordinate medication and treatment.
