Job summary

BoldAge PACE seeks a Physical Therapist to plan, organize, and implement therapy services for PACE participants. You will assess, plan, and provide treatment, teaching participants and families to maintain community-based support.

You will work within an interdisciplinary team, perform initial and ongoing assessments, and document care plans and outcomes. A license and valid driver’s license are required, with prior experience on an IDT preferred.

Qualifications

• Advanced degree in Physical Therapy from an accredited school
• Current license to practice Physical Therapy in the state
• Current driver’s license and proof of auto insurance

Responsibilities

• Provides high quality care as a member of the IDT.
• Performs in-person assessments for new enrollees and ongoing needs.
• Educates participants and families on home programs and safety.
• Trains staff in body mechanics and safety.
• Maintains documentation of evaluations and plans.
• Participates in quality improvement and care planning meetings
