Healthcare United is seeking a Nurse Practitioner (Geriatric Medicine) for a full-time role in the Bolton area of Connecticut, focusing on home-based care in patient homes and assisted living facilities. The position offers weekdays, with scheduling flexibility, and supports a multidisciplinary geriatrics team using Epic EMR.

Ideal candidates have a Family/Adult/Adult-Geron NP qualification, CT license or eligibility, and at least two years of postgraduate NP experience in geriatrics.

Qualifications

• NP training in Family, Adult, or Adult-Geron NP required.
• Unrestricted Connecticut license or eligibility.
• Minimum 2 years postgraduate NP experience, preferably in Geriatric Medicine.
• Current CV required.

Responsibilities

• Assess and manage older adults in home and assisted living settings.
• Coordinate treatment, referrals, and follow-up with a multidisciplinary team.
• Use Epic EMR to support continuity of care.
