Job summary

Qualifications

• Must hold an Ohio NP license and DEA registration.

Responsibilities

• Conduct health assessments to identify patient needs.
• Develop personalized care plans for geriatric patients.
• Regularly evaluate patient conditions and responses to treatment.
• Adjust treatment plans as necessary.
• Perform minor procedures and wound care as required.
• Educate patients and families on health management and prevention.
• Provide guidance on lifestyle modifications and self-care.
• Coordinate care with physicians, nurses and other professionals.
