Qualifications

Responsibilities

• Provide primary care in geriatric patients' homes and assisted living facilities.
• Perform routine physical exams and develop medical, educational, and psychosocial care plans.
• Coordinate referrals to other health team members or specialists as needed.
• Utilize EPIC for clinical documentation and patient care continuity.

Advanced Practitioners - Primary Care

Job Details:

• Full-time Nurse Practitioner (FNP, ANP, AGNP) or Physician Assistant position within the REACH (Resource for Elderly and Care at Home) Program
• Deliver compassionate primary care in the comfort of geriatric patients' homes and assisted living facilities
• Work within a multi-disciplinary geriatric medicine/primary care team in a home-based setting
• Full-time schedule, weekdays only
• Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
• Develop plans for further evaluation of health needs, including referrals to other health team members or specialists
• Provide appropriate treatment for problems within the scope of practice and refer appropriately when needed
• Utilize EPIC for state-of-the-art clinical and informatics technology to promote improved patient outcomes and streamlined continuity of care
• Unrestricted State of Connecticut Medical License required (or eligible)
• Minimum of two years of post-graduate experience as an APRN or PA required; Primary Care/Geriatric Medicine experience highly preferred
• New graduates with relevant nursing experience are encouraged to apply
