Job summary
BAS HealthCare in Farmington, CT seeks an Advanced Practitioner (Nurse Practitioner or Physician Assistant) to support the home-based REACH program, delivering geriatric care in patients' homes and assisted living facilities in the greater Manchester area.
The role requires an active Connecticut license (or eligibility) and 2+ years of post-graduate experience in geriatrics or related fields, with a comprehensive benefits package and CME opportunities.
Qualifications
- Unrestricted State of Connecticut Medical License required (or eligible).
- Minimum of two years of post-graduate experience as a Nurse Practitioner or Physician Assistant in Geriatric Medicine or a related field.
- Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA).

Responsibilities
- Deliver compassionate geriatric care in patients' homes and assisted living facilities in the greater Manchester, CT area.
- Work within a multi-disciplinary care team focused on Geriatric Medicine in the home setting.
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
- Develop plans for further evaluation of patient health needs, including referrals to specialists and other health team members.
- Provide appropriate treatment within scope of practice and refer appropriately when needed.
