Job summary

Qualifications

• Unrestricted Connecticut medical license or eligible.
• Two years of post-graduate APRN or PA experience required.
• Primary Care/Geriatric Medicine experience preferred.

Responsibilities

• Provide primary care in geriatric patients' homes and assisted living facilities.
• Perform routine physical exams and develop medical, educational, and psychosocial care plans.
• Coordinate referrals to other health team members or specialists as needed.
• Utilize EPIC for clinical documentation and patient care continuity.
