Job summary

Qualifications

• Bachelor's degree in social work, psychology, counseling, or a closely related human services field.
• Minimum of 2 years’ experience working with children and families.
• Proven ability to work independently in home/community settings while meeting deadlines.

Responsibilities

• Provide direct social work services to assigned clients/families in their home or natural environment.
• Develop comprehensive service and discharge plans based on client strengths and needs.
• Monitor progress and revise service plans as needed in collaboration with families and supervisors.
• Coordinate initial and ongoing assessments and perform case management and aftercare tasks.
• Advocate and connect families with formal and informal community resources.
• Demonstrate cultural competence and maintain professional relationships with families.
• Ensure safety of clients and communities and maintain compliance with program policies.
