Job summary

The Children’s Home Society of North Carolina seeks a practitioner to provide direct social work services to assigned client families under the Homebuilders model, delivering comprehensive assessments, goal planning, and evidence-based interventions in home and community settings.

You will collaborate with families, referents, and community resources, develop service plans, ensure safety, document progress, and maintain professional relationships while supporting families toward independence.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or closely related human services area.
• Minimum of 2 years’ experience working with children and families.
• Knowledge of child welfare policies and local community resources.

Responsibilities

• Provide direct social work services to assigned clients/families.
• Conduct holistic assessments including strengths, needs, and barriers to goals.
• Develop and implement service plans with measurable goals.
• Collaborate with families and referents to set intervention goals.
• Ensure safety and coordinate with community resources.
• Maintain professional relationships and advocate for services.
• Document progress and perform follow-up as required.
