Job summary

Pinnacle Family Services, part of Health Connect America, seeks a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities. You will work with children, adolescents, and families, guiding them through the FCT model with hands-on, relationship-driven care.

FCT Certification is required within a year; a Bachelor's in Human Services and QP qualifications help, and a valid driver's license is essential.

Qualifications

• Bachelor's degree in Human Services with 2 years' experience with youth/families or 4 years in a non-Human Services field with youth/families.
• FCT certification is a plus and pursuing it is supported.
• Must qualify as a Qualified Professional (QP) for youth/adolescents with primary mental health needs.
• Valid driver's license and access to insured private transportation required.

Responsibilities

• Deliver Family Centered Treatment services in home and community settings.
• Guide families through the four FCT phases, helping set goals and build skills.
• Coordinate with Treatment Team across medical, behavioral health, school, and community partners.
• Develop, implement, and refine individualized Service Plans with families.
• Connect families to community resources and aftercare supports.
• Maintain timely EHR documentation and participate in QA activities.
